When you are creating a new menu item for a form you need the form id number
you can find this by following this process
- Login to Joomla Admin
- Select Joomla Forms from the extensions menu
- Select the form you want to find the id of (the page reloads)
- Click the Preview Form link at the top of the page
- The form id is in the URL of the preview url form_id=1 where 1 is the form id :-)
